The Economic Opportunity Council of Suffolk under Hudson River Health Care (as a Lead Health Home in NYS) will be utilizing the OID for the EHR software used for member tracking and state reporting purposes. Our mission (EOC of Suffolk, Inc.) is to promote a goal of self-sufficiency by broadening the minds of children, revitalizing communities, and assisting families and children in need through the provision of services and to coordinate available federal, state, local and private resources.
