The Sayville Project of the School of Social Welfare of Stony Brook University provides care coordination services and supports to adults with chronic physical and behavioral health conditions in the south shore and central Suffolk communities of Long Island New York. As a care management partner of the Hudson River Health Care Health Home, the Sayville Project works in collaboration with other medical, behavioral health and social service partners to link and coordinate services for its clients.
