The OID will help uniquely identify Little Treasures-Petits Tresors, SLP & Psych, PLLC as a healthcare provider organization that can use coded systems for messaging and communication with our lead Health Home Agency. It will allow us to utilize our lead Health Home's computer programed system to assist us: * To enter data information such as (demographics, contact, personal information, service information, etc.) and allow us to generate electronic files for our members * To provide a platform for our Care Managers to document daily activities in real time and create detailed reports as well as uploading any consent forms and documents * To effectively communicate with our member's care team providers to better the member's quality of health * Track our member's medical diagnosis, medical history and updates * To customize Care Plans that address the needs and concerns of each of our members * To create and submit electronic claims for billing purposes * To be able to synchronize with MAPP, RHIO, and epaces * To be able to have Care Managers and the members provide e-signatures for required documents
