community-health-home-care — HL7 International OID Registry

Root OID for Community Health and Home Care. The Nurse Case Manager Program was designed for patients being discharged from the hospital or other care facilities; or patients, and their caregivers, who are interested in additional teaching and/or medical support in the comforts of their own home. This is a free service, funded by Care Compass Network. The Nurse Care Manager will make a combination of house visits and telephone calls to support the client to: 1. better understand medication and instructions for use 2. get connected with additional community resources 3. learn about the diagnosis and what to do if symptoms worsen 4. monitor blood pressure and other clinical measurements from home 5. follow up with family.
