San Diego Health Connect is a 501c3 community collaborative that started with the 2010 San Diego Beacon Grant awarded to San Diego from the Office of the National Coordinator (ONC). This developmental grant created a health information exchange (HIE) that allows the electronic interchange of patient medical care information including test results, imaging data, demographic data, allergies, medications prescribed and medical care summaries. Exchange occurs at the point of care between hospitals, physicians, clinics, and other care settings as needed to support care activities. Health participants contribute annual subscription fees to sustain SDHC.
